Please complete the top portion only of the Educational request form. While we try to accommodate every salons date requests, please understand that our educators work in salons also. Typically Mondays are the easiest days to schedule education. Please keep that in mind when filling out the attached form. Completion of the attached form is not a guarantee that we have an educator available for your requested dates and times. In such case we will contact you. In certain regions we are still searching for educators. If you are interested in joining our Salon Ware Educator team please contact us immediately.

Upon completion of the Educator Request Form please e-mail, fax or give the completed form to your Salon Ware consultant. Once we receive the completed form your consultant will contact you shortly to confirm your educational date!
